Please have your most current insurance card available each time you visit.
The world of medical insurance is becoming increasingly more complex. As PPO’s and other Managed Care insurance plans become more common, it is important that you know how your insurance works and what your responsibilities are under your insurance plan. If you are a member of a managed care plan, please confirm with your insurance carrier that we are part of your network. We belong to many of the area PPO’s and HMO’s. If you participate, please bring the referral form from your primary care physician with you to your appointment. We will assist you as much as possible, but we can only work within the limitations of the information you provide.
We are required by our contracts with insurers to collect copayments at the time of service. You are responsible for all deductibles, co-insurance and non-covered services. If you have any questions about your specific insurance plan, please ask and we would be happy to assist you. Surgical Fees/Deposits
We will contact your insurance company regarding eligibility and benefits and precertification requirements for your surgery. If you have an HMO insurance plan, a referral will need to be obtained prior to the surgery date. Our surgery scheduler will work with you, your insurance carrier and primary care physician to insure everything is in place.
Please note: We are only able to obtain benefits for our surgeon’s charges. The facility where the services are rendered will be billed separately, as well as any other ancillary services, such as anesthesia and pathology. Any questions regarding their fees or billing practices must be addressed with them.
If you have an unmet deductible, you may receive a call to review the quoted benefits and satisfy the expected contracted amount we have with your insurance company prior to the procedure date. Surgery Group S.C. accepts cash, checks, Visa, MasterCard, Discover, and American Express.

In-office Surgical Procedures:
Insurance eligibility and benefits are checked prior to office visits. In the event you are specifically coming to have an in-office procedure and insurance benefits identify an unmet deductible, you may be asked to pay a portion of the estimated charges at the time of service.
