The world of medical insurance is becoming increasingly more complex. As PPO’s and other Managed Care insurance plans become more common, it is important that you know how your insurance works and what your responsibilities are under your insurance plan. If you are a member of a managed care plan, please confirm with your insurance carrier that we are part of your network. We belong to many of the area PPO’s and HMO’s. If you participate, please bring the referral form from your primary care physician with you to your appointment. We will assist you as much as possible, but we can only work within the limitations of the information you provide.
We are required by our contracts with insurers to collect copayments at the time of service. You are responsible for all deductibles, co-insurance and non-covered services. If you have any questions about your specific insurance plan, please ask and we would be happy to assist you. We do accept Medicare assignment. As participating Medicare providers, we can file your Medicare claim for you. If you have supplemental insurance, please provide our office with a copy of the insurance card.
Surgical fees will be billed to your insurance carrier. We must have your insurance information in our office before surgery can be scheduled. We will contact your insurance company regarding eligibility and benefits for your surgery. Many surgical procedures require pre-certification. Our surgery scheduler will work with your insurance carrier regarding these requirements.
